Eczema là trạng thái viêm lớp nông của da, cấp hay mạn tính. Bệnh eczema không gây tử vong, nhưng gây ngứa ngáy, hoặc khô và căng da rất khó chịu. Bệnh thường xuyên tái phát. Như một số căn bệnh da khác, eczema cũng làm ảnh hưởng ít nhiều đến thẩm mỹ. Biểu hiện về lâm sàng của eczema rất đa dạng nhưng nói chung bao giờ cũng có đặc tính sau: ngứa, có mụn nước sắp xếp thành từng mảng giới hạn không rõ, tiến triển thành từng đợt, dai dẳng hay tái phát. Có thương tổn thuộc loại xốp bào.
Yếu tố nguy cơ gây bệnh eczema
Hai yếu tố cơ bản phát sinh eczema là cơ địa dị ứng và tác nhân kích thích ở trong hay ngoài vào cơ địa ấy.
Do cơ địa: Có tính chất gia đình, di truyền, tiền sử trong gia đình có người bị chàm, dị ứng, hen suyễn. Các tác nhân kích thích bên trong kèm theo có thể bị viêm mũi xoang, xơ gan, viêm đại tràng, viêm tai xương chũm, các bệnh về thận...
Do các dị nguyên: Việc dùng các thuốc như: lưu huỳnh, thủy ngân, thuốc tê, sunfamid, chlorocid, penicillin, streptomycin... là lý do thúc đẩy eczema tiến triển. Mặt khác bệnh eczema cũng phát sinh khi tiếp xúc với các hóa chất như: xi măng, thuốc nhuộm, nguyên liệu làm cao su, sơn xe, dầu mỡ, than đá, phân hóa học, thuốc trừ sâu, axit, kiềm,... Các sản phẩm vi sinh có cơ chế dị ứng: vi khuẩn, nấm, siêu vi. Các yếu tố môi trường sống: khói, bụi, lạnh, nóng, ẩm, mặc trang phục được làm từ những chất liệu như len, vải được dệt không được mịn màng... Yếu tố tâm thần kinh cũng ảnh hưởng lên bệnh này, vì thế với một số người eczema cũng có thể nặng lên sau những chấn thương tâm lý, stress, lo âu căng thẳng.
TRIỆU CHỨNG
Bệnh phát triển qua 4 giai đoạn: đỏ da, mụn nước, lên da non, liken hóa (hăm cổ trâu). Bệnh có thể trở thành mạn tính, da dày lên, ngứa gãi nhiều. Nhiều trường hợp bội nhiễm gây viêm da mủ, hoặc nhiễm khuẩn rất nặng.
Các thể lâm sàng của bệnh Eczéma
Eczema có nhiều hình thể tùy theo giai đoạn như:
* Chàm dạng nấm (eczema rubrum): Thương tổn khu trú ở chân, màu đỏ sẫm, trên có mụn nước lấm tấm.
* Chàm dạng viêm quầng (eczéma érysipélatoiđe): Thương tổn khu trú ở mặt, sưng đỏ mi mắt.
* Chàm tổ đỉa (bệnh tỗ đỉa): Thương tổn khu trú ở rìa ngón tay, ngón chân, có the cả lòng bàn tay bàn chân, mụn nước từ sâu đội mặt da, rất ngứa.
* Chàm vẩy mảng (eczema lamelleux): Thương tổn thường khu trú ở chi dưới, vẩy to thành mảng, dưới vẩy màu đỏ chảy nước vàng, diễn biến dai dẳng.
* Chàm rừng (eczema corné): Thương tổn khu trú ở lòng bàn tay, chân, vẩy dày trắng, nứt nẻ, sưng đau, dưới vẩy có mụn nước.
* Chàm thể đồng xu (eczema nunmulaire): Có hình tròn hoặc hầu dục màu đỏ hồng, trên có mụn nước vẩy tiết, khỏi từ giữa, khu trú ở các chi.
Biến chứng bệnh Eczéma
* Hằn cỗ trâu: Dày da, bóng, màu nâu xám.
* Chàm chốc hóa (eczema impétiginisé) do nhiễm khuấn phụ, ngoài mụn nước kèm theo bọng nước có mủ.
Có thể chia làm hai loại eczema: eczema khô và eczema ướt (khi thương tổn là những mụn nước, hoặc đang rỉ dịch, rất ngứa và dễ bội nhiễm). Những người có biểu hiện eczema khô thường nứt nẻ, xuất hiện ở bàn tay, bàn chân, nặng lên khi trời lạnh, hoặc khi tiếp xúc hóa chất, xà bông, chất tẩy rửa...
ĐIỀU TRỊ THEO PHƯƠNG PHÁP DRCUONG
Bệnh eczema không thể điều trị dứt hẳn được, đây còn là một vấn đề khó khăn. Việc điều trị là nhằm kiểm soát các cơn ngứa, giảm các biểu hiện viêm da, ngăn ngừa, hay trị liệu tình trạng bội nhiễm nếu có, làm giảm thiểu sự xuất hiện của những thương tổn mới trên da. Phải tùy theo tuổi và tổn thương của bệnh mà có cách điều trị phù hợp. Giải pháp điều trị tốt nhất cho bệnh eczema là tích cực tìm ra nguyên nhân gây bệnh để tránh hoặc hạn chế tiếp xúc kết hợp dùng thuốc uống với thuốc bôi ngoài da theo chỉ dẫn của bác sĩ.

Bì khang vương ngày 2 lần, mỗi lần 6 viên, uống sau ăn.
Các thuốc bôi ngoài da
Thuốc mỡ Bì khang vương chủ yếu dùng trong giai đoạn eczema bán cấp và mạn tính. Thuốc thuẩn thảo dược có thể bôi diện tích rộng ( khuyến cáo mới bôi nên bôi vùng nhỏ xem không có phản ứng có thể bôi rộng, cho đến hiện nay chưa ghi nhận có biến chứng bôi trên da – thuốc 100% thảo dược không có Cocticoide).
Để điều trị bệnh eczema hiệu quả, người bệnh cần đồng thời áp dụng các biện pháp phòng bệnh như: Bệnh nhân cần uống nhiều nước mỗi ngày, có thể thay nước lọc bằng các loại trà thanh nhiệt (actiso, hoa hòe, hoa cúc,...), nước ép trái cây tươi chứa nhiều vitamin để giải độc cơ thể, bài trừ độc tố, nâng cao sức đề kháng, ăn thức ăn lỏng nhẹ. Tránh dùng rượu, bia, thuốc lá, cà phê, hải sản, đồ hộp, thức ăn sống, lên men, các thức ăn chế biến có nhiều gia vị cay nóng. Giữ vệ sinh da sạch sẽ, đặc biệt là vùng da bị bệnh. Tránh cọ xát, gãi, xát xà phòng vì sẽ làm vùng da bị bội nhiễm tạo nên những tổn thương khó lành. Có thể dùng chè tươi, lá bàng tươi nấu lấy nước để tắm. Bệnh nhân eczema nên tránh tiếp xúc với các nguyên nhân gây dị ứng, dễ làm bệnh nặng thêm.
